Spanish-Brazilian Rodrigo began the Leeds recovery by scoring after 65 minutes. On 81 he missed a penalty when defender Joel Bagan saved a shot on goal by Dominican Junior Firpo at arm’s length and received a red card.
Perkins, 18, came to Rodrigo’s defense with a shot on goal that sealed the tie and necessitated a tiebreaker.
The last time these teams faced off in the tournament was in 2002, when Leeds, the then Premier League leaders, were surprised by a Cardiff who were 10th in the Third Division.
Also on Sunday, Derby County became the first qualifiers for the fourth round thanks to a 3–0 win over another Third Division team, Barnsley.
The duel between two Second Division clubs, Bristol City and Swansea, ended 1–1, so both would have to play a tiebreaker in the third round.
There are eight meetings in total on Sunday, headlined by two of the Premier League giants. Manchester City will host Chelsea.
If the Premier League leaders beat Oxford on Monday, the winner will face Arsenal.
Aston Villa receive Stevenage from the Fourth Division, from the top flight.
